Plan Overview
In creating this plan, Minneapolis and Hennepin County joined more than 300 cities across the US that are striving to change the way their communities address homelessness.
Who: The Commission to End Homelessness, comprised of nearly 70 Hennepin County leaders – representing federal, state, and local governments, businesses, nonprofits, faith and philanthropic communities, along with homeless and formerly homeless citizens – spent 100 days building the plan to end homelessness.
What: Heading Home Hennepin is a 10-year plan to end homelessness based on proven models and national best practices. The report contains 6 broad goals, 30 recommendations, and over 50 concrete action steps to end homelessness. We endeavor to end homelessness by developing permanent new supportive housing opportunities, enhancing the collaborative prevention efforts of government, community organizations, and social service agencies, and providing constant outreach to all individuals in Minneapolis and Hennepin County experiencing homelessness. Successful implementation of the plan will not only prevent people from falling into homelessness whenever possible, it will move people off of the streets and out of the shelter system cycle, give them stable housing along with the support services they need to succeed, and drastically reduce unnecessary, expensive costs to taxpayers.
Where: Heading Home Hennepin focuses on ending homelessness across the City of Minneapolis and throughout Hennepin County.
When: By the year 2016, Heading Home Hennepin expects to reach its goal of providing safe, decent and affordable housing for all people in Minneapolis and Hennepin County, as well as ensuring that everyone has access to necessary services and no one slips into homelessness.
